At Peach Tree Medical Center, our main concern is for your health and wellness. As your medical home, our goal is to build a relationship between you and your healthcare team. We strive to provide all patients with well- rounded and coordinated healthcare. You and your health are the primary focus of your healthcare team at Peach Tree Medical Center.


Our Role As Your Medical Home: 

  • Provide preventive care and complete health information

  • Provide best practice care; care that offers patients the best outcomes for their condition

  • Keep track of your labs, x-rays, referrals, and medications

  • Coordinate specialty care if needed

  • Remind you of important health screenings

  • Help you set healthy goals and help you track your progress toward meeting those goals

  • Partner with you in making your healthcare decisions

  • Coordinate care with you, your authorized representatives, and other healthcare providers

  • Keep you informed and provide education as needed or requested


Your Role As Our Patient:

  • Communicate closely with us
  • Keep us up to date with your medications, immunizations, allergies, conditions, tests, consultations, and hospitalizations
  • Advise us of any changes about you and your families' medical histroy
  • Inform and authorize your other providers to coordinate with us
  • Participate in decisions about your health
  • Follow your provider's instructions
  • Ask questions!


In a medical home organization, patients are encouraged to choose their primary care provider. Our staff will try to schedule all your appointments with the provider you selected. The advantage for you is that you have a provider you like working with and any health issues are followed in a timely fashion. 

There may be times when it is in your best interest to see a different provider for timeliness or urgency, and your primary care provider will have updated information about those visits to ensure that you receive continuity of care. 



Protect Your Skin

Skin cancer is the most common cancer in the United States. Most cases of melanoma, the deadliest kind of skin cancer, are caused by exposure to ultraviolet (UV) light. To lower your skin cancer risk, protect your skin from the sun and avoid indoor tanning. Follow these easy options. Stay in the shade, especially during midday hours. Wear clothing that covers your arms and legs. Wear sunglasses that block both UVA and UVB rays. Use sunscreen with SPF 15 or higher and both UVA and UVB protections. Avoid indoor tanning. Patient Portal can now be launched via an App downloadable on Google Play and the Apple App Store. Through the Patient Portal App, patients can communicate with our doctors, access medical records and test results, and request appointments and medication refills.
